Prior to your first consultation all we need is for you to complete a Medical & Dental History Form. This can be collected from our surgery or downloaded from the Web Site or filled in electronically.
We offer a Belwell Dental Membership Plan which includes Comprehensive Dental Examinations twice a year, Two Hygiene visits and Routine Bite Wing X-Rays. Other benefits include Worldwide Dental Trauma Insurance, Emergency Call Out Insurance and Redundancy Protection. Membership also benefits patients with 10% of all Clinically Necessary Dental Treatment.
At your first appointment, you will be expected to pay for your basic examination charge. Any further treatment that you may require and have requested will be discussed with you and you will be given an estimate prior to any charges. Payment for treatment will then be collected subsequently over the course of treatment - you will be informed how much is due each visit depending on your treatment plan.
If you are an existing registered NHS patient, you will be liable for the current NHS charges for examinations and treatment. If you are exempt from paying, we would ask that you bring your evidence of exemption to all visits.
If you have a dental emergency and you are a registered patient with us, we will endeavour to see you the same day where possible. If you have an emergency out of hours that cannot wait till the surgery re-opens, we have an out-of-hour’s emergency service available.
